Documents You Need to Bring

To ensure a smooth visit, please bring the following:

  • A valid government-issued photo ID (passport, driver’s license, or state ID).
  • Your vaccination records (if available).
  • Proof of address (such as a recent utility bill, lease, or mail showing your current residence).
  • Any previous medical documentation related to chronic or ongoing conditions.
  • Form I-693 (we can provide one at the clinic if you don’t have it).

Important: Form I-693 must be submitted with your Form I-485 application. We’ll provide your completed exam in a sealed envelope ready for submission.
